Laks and Zoe Supportive Home does not discriminate on the basis of race, religion, creed, or color. It is the purpose and goal of our home to provide residential l care to adults between the ages of 18 and 59 years of age who are in need of non-medical care and supervision.

Documentation shall include:

● Admission Agreement

● Refund Policy

● Needs and service plan

● Functional capabilities assessments

● Information regarding individual likes and dislikes, abilities, interests, and activities

● History of aggressive or dangerous behavior of the client towards self or others

● Identified needs for training and treatment activities

● Identified medical needs, including dietary requirements

● Name, address, and telephone number of the client's authorized representative

● The name, address, and telephone number of actively involved parents or relatives. 

Laks and Zoe Supportive Home serves a maximum of 4 clients. 

Hours of operation are 24 hours per day, seven days per week.
